Team Organizational Meetings
The purpose of the team organizational meetings is to collect the necessary information to get players rostered and outfitted to play. Players are not required to come to these meetings, however, you may want to bring them to ensure proper fitting of uniforms. Uniform samples will be available.
Please find the date, time and venue of your team meeting below and make plans now to attend. If you cannot attend at the scheduled time, please come to a similar age/gender meeting to complete the necessary paperwork. If your schedule does not permit you to attend any of these meetings, please contact the office and make arrangements to stop by and complete the necessary forms, pay the required deposit and order and pay for your player's uniform.

The meetings will be held at Sylvania Senior Center (Map), just west of Tam o'Shanter, 7140 Sylvania Avenue in Sylvania. If you come in from the parking lot on the Western side, take a left after coming through the doors.
Please come prepared with the following information and forms to expedite the process:
- One Copy of Player Birth Certificate (to be left with club)
- Two (1 inch) Player Photos (small school photo works well)
- Date of last tetanus booster
- Physician's name and phone numbers
- Insurance carrier name, contact info and policy#
- Emergency Contact Information
- Credit Card Info - for U10 players and up (for Tournament/Hotel reservations)
You will also need to bring your checkbook to pay the required deposit:
U8 - $100.00
U9 and 10 - $125.00
U11 and U12 - $150.00
U13 and U14 - $175.00
Uniform payments ($170.00 - $185.00) are required for uniform orders. We will accept Visa and Mastercard payments for uniforms.
To expedite the process, you may download each of the following forms here, complete them in advance and bring them with you to the meeting.
- Medical Release Form Required from all players (must be notarized)
- OYSAN Player Registration Required from all players
- US Club Player Registration Required from all players
- MI Premier Player Registration Required from all U13 & U14 players
- NWOYSL Player Registration Required from all U8 - U12 players
If at all possible, please have the Medical Release form notarized. We are working to have a Notary Public on site, for your convenience, but do not have all sessions covered. Please do NOT sign your forms until you're in the presence of the Notary.
